The Pediatric Transitional Care Program at South Davis Community Hospital was developed in collaboration with the University Of Utah Department Of Pediatrics.
Pediatric transitional care bridges the gap in healthcare systems for children with special healthcare needs. It provides a natural transition for children from acute care hospitals and pediatric intensive care units to home and community settings.
