The pediatric transitional care and specialty care program is developed in collaboration with the University of Utah’s Department of Pediatrics to provide a natural transition for children from acute care hospitals and pediatric ICUs to home and community settings. SDCH is Utah’s leading provider of post-acute, transitional, and rehabilitative care for children with medically complex and technology dependent health care needs. Patients come to us with vents, trachs, needs for rehab from accidents, and with birth-related medical complexities. Our program brings children here who are in need of continued skilled care while we train and prepare the family to care for them at home.
